Eligibility: In order to be enrolled in Case Management services an intake has to be completed

Our professional and highly trained staff coordinates customized treatment for consumers served. Services provided through Ryan White include:

• Primary Medical Care
• HIV Medication
• Rental Assistance
• Food Pantry
• Transportation
• Utility Assistance

Referrals to: Dental Care, Mental Health Services, and Legal Services
